DESCRIPTION OF WORK

As part of the care team, you will conduct psychosocial assessments during admission and regularly thereafter, tailoring interventions to fit each resident's specific needs and preferences. You will participate in team meetings for your assigned residents and ensure that all documentation meet standards, as well as those of accrediting bodies. Completing the necessary parts of the resident assessment instrument minimum data set (MDS) is required upon admission and at set intervals. You will also carry out behavioral assessments and implement person-centered strategies, aligning each resident's psychosocial evaluation with their care plan. Providing direct social work services, you will address behavioral and psychosocial issues through counseling and management techniques. You will support residents and their families with various treatment methods, helping them identify challenges and develop strategies for better functioning. Additionally, you will evaluate residents' readiness for discharge and collaborate with them, their families, and community contacts to create a thorough discharge plan, ensuring smooth transportation and transition. Building positive relationships with colleagues while being aware of team dynamics is essential.

Work Schedule and Additional Information:


  • Full-time employment, 75 hours bi-weekly
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • A bachelor's or master’s degree in social work or social welfare.
  • Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.

Other Requirements:

  • You must be able to perform essential job functions.

Legal Requirements:
  • A conditional offer of employment will require a medical examination, and drug screening.
  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
