Job Description

Salary Range:  $57.9800 - $89.1700 /hour. Actual compensation may vary based on geographic location, work experience, skill level, and education.

 

Primary Duties and Responsibilities 

The Clinical Navigator is a Physical Therapist responsible for the seamless coordination of care across the continuum for patients with complex diagnosis under the umbrella of brain injury. The navigator is a patient advocate who coordinates care in conjunction with the physicians from time of diagnosis or admission through all continuum of care settings. Educates patient and family/significant others on the health care experience; implements, coordinates, monitors and evaluates care options, services, and resources, access evidence-based treatment. The Clinical Navigator also assists in quality improvement initiatives by gathering, tracking, and researching quality metrics. Monitors outcomes, e.g. patient satisfaction, compliance with treatment regimen and loyalty. Interacts with other staff in the department and may orient and train incoming staff, collaborates with Physicians/surgeons for referrals. Conducts outreach and identification of patients, conducts initial screening, evaluation, and interventions with accompanying physical therapy plan of care, introduce patients to brain injury program and services. Able to manage multiple patients at the same time and provide care in multiple clinical settings. Coordinate the schedules of multidisciplinary team members to ensure timely and efficient care delivery. 

 

The navigator will assist the operations team in the development, implementation, and evaluation of program policies, procedures, and protocols. Participate in program planning, assist operations team in resource allocation to ensure that the program meets its goals and objectives. Coordinate meetings, preparing agendas, and documenting minutes to ensure effective communication within the team. Maintain accurate patient records, ensuring that all documentation meets regulatory and professional standards. Collect and analyze data related to patient outcomes, treatment effectiveness, and program performance. Prepare regular reports on program metrics, patient outcomes, and quality improvement initiatives for review by leadership. Assist with the orientation and training of new staff, ensuring they are familiar with the program’s protocols and procedures. Act as a liaison between clinical and administrative staff to ensure seamless communication and collaboration. Ensure the program complies with all relevant local, state, and federal regulations, as well as accreditation standards. Assist in the preparation for audits, inspections, and accreditation reviews by maintaining organized and up-to-date records. Implement and monitor compliance with patient privacy and confidentiality regulations (e.g., HIPAA). Assist the operations team in managing program resources, including supplies, equipment, and facilities, to ensure they are available and in good condition when needed. Assist with the procurement and inventory management of clinical and administrative supplies. Coordinate with vendors and external service providers as needed to support program operations. Performs other duties as assigned.

 

Education and Experience

  • Five years’ experience as a Physical Therapist in an acute healthcare system environment required. Previous experience with Microsoft Office and healthcare database systems.

 

Preferred: 

  • A minimum of 3 years of experience in a field of brain injury. Doctoral degree in Physical Therapy (DPT). 
  • Bilingual or multilingual. 

 

Licenses Required 

  • Current licensure as a Physical Therapist in the State of California. 

 

Certifications Required 

  • Current BLS Certification required. 

 

Certifications Preferred 

  • Advanced Board Certification in area of expertise.
