About the Role - MDS Coordinator
As an MDS Coordinator , you'll play a key role in our success by overseeing the MDS process and collaborating with an interdisciplinary team to deliver exceptional care. Your expertise will ensure compliance with federal and state regulations, maximize reimbursement opportunities, and uphold the highest standards of care for our residents.
Responsibilities:
* Manage and complete MDS assessments accurately and on time.
* Ensure compliance with all federal, state, and facility policies.
* Collaborate with the interdisciplinary team to create individualized care plans based on resident assessments.
* Facilitate care plan meetings to address residents' needs.
* Audit documentation for accuracy and compliance with CMS regulations.
* Provide staff education on MDS processes, documentation, and quality reporting.
* Drive facility reimbursement by optimizing MDS data submissions.
About You
You're a detail-oriented, team-focused professional with a strong commitment to resident care. Your leadership and expertise will make you an invaluable part of our team.
Qualifications:
* Licensed RN or LPN.
* Minimum 2 years of MDS Coordinator experience or a similar role in long-term care/skilled nursing.
* In-depth knowledge of MDS 3.0, RAI process, and care planning.
* Familiarity with Medicare, Medicaid, and reimbursement processes.
* Excellent organizational, time-management, and interpersonal skills.
