The Care Manager (CM) is the key point of contact for TheKey's clients, their families and support networks. The Care Manager is responsible for developing and maintaining the client care plan, supervising caregiver teams and managing the client relationship. They must have a pleasant and engaging personality, be able to remain calm in challenging situations, be able to think clearly and quickly implement solutions to problems as they arise. Finally, given the constant interaction with clients, their families, and Caregivers, the Care Manager must be a professional in knowledge, action, and appearance – always reflecting the desired image of TheKey.

This is a full-time position. The Care Manager will need to be responsive to and regularly available to the needs of our clients – including some time during evenings and weekends.

Job Responsibilities:

  • Client Intake
    • Perform initial and detailed assessments of clients either alone or in conjunction with a Client Care Consultant or other TheKey staff member.
    • Create an appropriate care plan, client specific orientation and caregiver training requirements for assigned clients.
    • Create in-home binders for all clients, including medication sheets and supplemental charting requirements.
    • Coordinate the necessary equipment and supplies required by each client.
    • Participate in determining the appropriate caregiver(s) for each client.
    • Serve as the initial caregiver for clients as needed.
  • Caregiver Training
    • Meet with all new members of a client’s caregiver team to provide an orientation to the client, review the care plan and train on client specific needs.
    • Conduct a verbal review (telephone or in-person) with each caregiver within 1 business day of completing their first shift with a client.
    • Identify any additional training needs for caregivers assigned to clients and ensure completion.
    • Review Caregiver performance and client satisfaction following the start of service (or as necessary) to identify issues and make the appropriate corrections.
  • Care Management & Client Retention
    • Conduct thorough in-home reviews on a regular basis (preferably in conjunction with new caregiver training or supply delivery) per TheKey's Home Visit schedule, to identify issues or opportunities and to ensure client satisfaction.
    • Ensure client binder is updated as necessary and perform any necessary training for Caregivers on care plan changes.
    • Communicate consistently and professionally with clients and/or clients’ families, medical teams and support networks to maintain quality care and satisfaction with TheKey services.
  • Miscellaneous
    • Serve as back-up Caregiver for clients as necessary.
    • Assist with other projects/tasks as assigned.
    • Assist with Referral Marketing by joining on specific calls/meetings.

Job Qualifications:

  • Bachelor's/Associate degree preferred  
  • Experience in elder care, team members supervision, customer service
  • Strong computer skills including Microsoft Office programs
  • Ability to work under pressure and manage multiple demands in a timely fashion
  • Excellent written and verbal communication skills
  • Experience in elder care or in-home care highly preferred
